Under "Name" please format as "BOOKSTORE, Bookseller's Name"

ie. "BOOKSTORE, Andrea Andersen"

Please clearly list an email and phone number for your bookstore.

Under "Comment," please list the exact titles and quantity of each you'd like to order from Andrea, at a wholesale discount.

You will be emailed a Shopify invoice. Please note, your delivery will arrive between 3-4 weeks.
